Whole person refers to physical health, mental health, and lifestyle choices that impact overall wellness. Services are provided by an Integrated Health Home Team which includes a nurse care manager, a care coordinator, and peer support specialists. The service may include intensive case management for clients receiving Habilitation services under a 1915 waiver.
